Resizing elements is as desired, 
depending on the size you make your tag

Open new canvas 800 x 800

Paste paper 8
Apply mask
Merge group

Paste frame 27
Click inside frame 
Expand by 5
Invert
Paste paper 10
Delete
Paste close up mirrored tube
Delete
Select none
Change blend mode to screen

Paste tube
Place at right

Now add elements of choice

I removed a piece of the cup to make it look more like "chip"

Using pen tool draw out a curvy line

Type text out that you want

I used

Once upon a time, in a faraway land, a young prince lived in a shining castle...

Convert to raster after hiding pen tool layer

Float - Expand by 1

Fill new layer with white and move below text layer

Merge together

Add name

Add drop shadow to layers

Add CR info

Crop & Save
