Okay, let's get started.
Remember to save often!!!

Open my template and shift D
Close original 

Move the word art to left  side of black rectangle

Move the lips to the right bottom corner of rectangle

Paste your tube

Duplicate

Move to left side of black rectangle

Duplicate

Mirror

Merge both together

Float black rectangle layer

Invert

Click delete on merged tube layer

Lower opacity to 36

Apply scan lines changing spacing to 2

On main tube place in center of tag

Duplicate again

Float circle layer

Invert

Click delete on bottom tube
Select none
Crop bottom area on top tube
Merge both together

Add name
I applied noise and gel effect to mine

Add drop shadow to layers of choice

Add copyright info

Save
Make sure you close white bg and crop if needed
