Okay, let's get started

Remember to save often!!!

Resizing elements is as desired, 
depending on the size you make your tag

Open new canvas 800 x 800

Use my tag if needed as guide

Paste preview paper 

Apply shower door filter twice

Paste frame element 12

Click inside frame and expand by 10

Invert

Paste paper 1

Delete

Paste close up mirrored tube on right

Delete

Select none
Change blend mode to overlay

Duplicate and erase all but eye area

Change blend mode to hard light and lower opacity to 60

Paste tube and move to left

Now add other elements of choice

Add name using alpha letters

I pasted element 50 at very bottom of alpha so they look like in snow

Add drop shadow to layers

Add CR info

Crop & Save
